This week, we're turning to a condition that gets diagnosed later, dismissed more often, and treated with far less confidence:
Female Alopecia.
We've touched on hair loss in women before, but today we're going deeper.
Beyond "it's probably stress," into what actually drives female pattern hair loss and how to recognize, diagnose, and manage it with confidence.
Often chalked up to age, hormones, or "just genetics," female pattern hair loss (FPHL) is a distinct clinical entity with its own presentation, its own workup, and its own treatment ladder, one that looks meaningfully different from the male playbook.

What is Female Alopecia?
Alopecia in women is frequently misread as a single problem when it's really several, each with a different pattern and a different workup.
The leading type is female pattern hair loss, affecting an estimated 40% of women by age 70, but the presentation is easy to miss because it rarely looks like the receding hairline clinicians are trained to spot in men.
But clinicians must also recognize the conditions that mimic it, and the systemic causes that can sit underneath it.
Clinical Features of Female Alopecia

That's usually normal (most people shed 50–100 hairs a day as part of the natural growth cycle).
The red flag comes when shedding is excessive, sudden, or patterned, and in women, "patterned" rarely means a receding hairline. Here's what to look for:
Female Pattern Hair Loss (FPHL):
Diffuse thinning centered on the part line, with a widening part ("Christmas tree" pattern) and a frontal hairline that is usually preserved, unlike androgenetic alopecia in men.
Telogen Effluvium:
Diffuse, generalized shedding, often triggered by postpartum hormonal shifts, illness, crash dieting, surgery, or acute stress. Typically self-limiting once the trigger resolves, but can uncover or accelerate underlying FPHL.
Alopecia Areata:
Sudden, well-defined bald patches. Look for "exclamation point hairs," short, broken hairs that taper at the base.
Traction Alopecia:
Gradual thinning at the temples and hairline margin from chronic tension, tight ponytails, braids, weaves, or extensions. Reversible early; can scar if the tension continues unaddressed.
Frontal Fibrosing Alopecia / Scarring Alopecia:
Progressive hairline recession with loss of follicular openings, redness, or scaling at the margin, disproportionately seen in postmenopausal women and always a reason for prompt referral, since scarred follicles do not regrow.
Diagnostic Approaches: Beyond "It's Probably Hormones"
The workup for female hair loss earns its reputation for being frustrating because it's so often skipped or shortcut. A careful history and a short panel usually sort the picture:
Ferritin and iron studies: Iron deficiency, even without frank anemia, is a well-documented driver of diffuse shedding in women and is frequently missed if only a CBC is checked.
TSH: Both hypo- and hyperthyroidism can present as diffuse hair loss.
Signs of hyperandrogenism: Irregular cycles, acne, hirsutism, or rapid-onset thinning warrant screening for PCOS or, in more abrupt or severe cases, an androgen-secreting source.
Medication and life-event history: New medications, postpartum timing, recent illness, surgery, or significant weight change all point toward telogen effluvium rather than FPHL.
Scalp exam: Miniaturized hairs and a widening part favor FPHL; perifollicular redness or scarring changes the referral urgency entirely.
